Initial Comments:
Per receipt of correspondence dated March 1, 2024, the provider agency has requesting closure of the above location as an ICF/IID residence, to be effective February 2, 2024. The purpose of this action is to discontinue participation under 42 CFR, Part 483, Subpart I Regulations for Intermediate Care Facilities for Individuals with Intellectual Disabilities. The total number of beds currently certified under the location totals five.
All applicable materials have been forwarded to the Central Office of the Division of Intermediate Care Facilities for processing on March 13, 2024 to the Office of Long-Term Living, Department of Human Services. This action will be effective February 2, 2024, the date requested in the provider correspondence.
